K&D Transitions ltd. is a organization that provides person-centered support, transitional housing and programs that develop independent living skills to youth struggling with mental health or living with a cognitive disability. Through our various housing models, we are able to tailor appropriate living environments and programming for youth from all walks of life that aid in their emergence to independent living.
Find out more about our transitional programs that are offered in to participants living in our 2 unique living situations. These living situations are 24 hr staffed community homes and 24 hr staffed apartment units.
