Who We Serve

K&D Transitions ltd. is a organization that provides person-centered support, transitional housing and programs that develop independent living skills to youth struggling with mental health or living with a cognitive disability.  Through our various housing models, we are able to tailor appropriate living environments and programming for youth from all walks of life that aid in their emergence to independent living.  

Our voluntary services are offered to youth or adults receiving funding through various Provincial CFS Agencies. The first step to potentially receiving services is our referral form which will require the submission of pertinent supporting documentation by the funding agent or Social Worker and reviewed by agency Directors and Case Management team. 

Once a referral form is reviewed, which includes a comprehensive review of past responses to service providers, support, and factors such as psychiatric diagnoses and history of unsafe behaviour (e.g., sexual or physical inappropriate behaviour or violence), an Emerge Program meeting will be set up with the referred individual. 

The meeting will determine if the services and programs we provide is a good fit and to ensure the person being referred has a desire to “take that first step forward”. Additionally, the Emerge Program meeting is an opportunity for the referred individual to share some insight about themselves, their goals, history and desire to engage in a process of change. 

Once the meeting has been completed, the team will meet again to review the meeting notes and then confirm one last time that everyone is in favour. 

Once confirmed, a proposal for services is then sent to the funding agent and services begin upon approval.
